Pre-Visit Knowledge:
• Our office is located at 1177 PA-315, Dolphin Plaza, Wilkes Barre, PA 18702
• Free parking Is available directly outside of the office.
• Please arrive 15-20 minutes early for your first appointment to complete any remaining paperwork.
• If you complete your paperwork before your visit, it will help speed up the check-in process.

Items to Bring:
• Photo ID
• Insurance card(s)
• Referral (if required by your insurance)
• Current medication list
• Any recent imaging (MRI, CT, X-rays) or reports if they were performed outside of our office
• Previous pain management records, if applicable.

Initial Steps:
• Schedule your consultation by phone
• Complete your new patient paperwork before your appointment, if possible
• Our staff may contact you before your visit to confirm your appointment and answer any questions

First Impressions:
When you arrive, you’ll be greeted by our front desk team ad checked in. A medical assistant will review your medical history, medications, and concerns before you meet with one of our providers. During your consultation, your provider will discuss your symptoms, review any imaging or records, perform an evaluation, and develop a personalized treatment plan.

We strive to provide individualized compassionate care for every patient.

Please bring any questions or concerns you have – we want you to feel informed and comfortable with your treatment plan. Depending on your visit, additional testing or procedures may be recommended, but not all treatments are performed on the first visit.
